Prepayment is required. Reprints will be provided when the paper has been published in a journal and the prepayment has been received. 

Reprints may be ordered in multiples of 100. Minimum order is 100 copies. Domestic prepaid orders under $250.00 incur no shipping and handling charges. All billed orders and orders over $250.00 incur the additional shipping and handling (S/H) charges as shown in order form.
