Preliminary Reports
The Preliminary Report must be sent to Nicole Alston at NY Headquarters or the district designee, postmarked by November 1.

Monthly Reports
The Monthly Reports must be sent to Nicole Alston or district designee, postmarked by the fifteenth of the month following the month for which the report is made. Attach the February report to the Final Report.

Final Report
The Final Report must be sent to Nicole Alston or district designee, postmarked by March 15 or two weeks prior to the first day of the district's SPDC, whichever occurs first. A late final report carries a penalty of ten points/day based on the postmark. The points are subtracted from the baseline points earned. This required report should include a letter of transmittal, the completed Summary Report Form, the completed supplements, and appropriate documentation for Categories 1,5, 6, 9 and 10. The section is discouraged from adding information that does not bear directly on establishing the point count.
